A

accidental death and dismemberment (AD&D) rider

A supplement to many life insurance policies that provides an additional cash benefit to the insured or his/her beneficiaries if an accident causes either the death of the insured or causes the insured to lose any two limbs or the sight in both eyes.


Alumni

Assured Access
As a recent graduate, you will be assured access for coverage when you apply for the MedOne Plus® $2,500 deductible insurance plan design as long as you:
     
  • apply within 90 days of your graduation date, proof of graduation required
  •  
  • are 29 years old or younger at the time of application for insurance
  •  
  • are replacing a current health insurance plan with this new coverage
Plan Design Options
MedOne Security is a great option if you are looking for lower premiums. There are three plan designs to choose from with $1,000 or $2,000 deductibles and a $40 co-payment.

MedOne Plus® allows you to choose an insurance plan that will meet your individual needs as well as your budget.  Again there are three plan designs to choose from with deductibles of $1,000, $2,500, and $5,000.   Plus, you can add a $40 co-payment to the $1,000 deductible plan design.

Coverage available for an alumni's spouse and dependent children.


Alumni - California

Are you a recent graduate?
Recent graduates of participating Alumni Associations and their dependents who enroll within 90 days after graduation will be approved automatically (final rates may vary) for the Blue Shield of California plans, provided the following:
  • your application
  • your payment
  • your proof of graduation (graduation confirmation letter, commencement bookletintact, Student Verification Transcript or diploma)
All must be received in Park Ridge Illinois within 90 days of your actual graduation date.

Annual Deductible

A specific dollar amount that you may be required to pay out-of-pocket each year before your health insurance plan starts to make payments for your claims.  Not all plans have a deductible.  In general, most Indemnity and PPO plans have a deductible, while HMO plans do not typically require one. Copays typically do not count towards a plan's deductible.


Annual Out-of-Pocket Maximum -- Individual

A dollar amount set by the plan which puts a cap on the amount of money the insured must pay out of his or her own pocket for covered expenses over the course of a calendar year.
